How Do Doctors Diagnose Inflammatory Bowel Disease (IBD)?

Getting an accurate diagnosis is really important because a few other conditions can look a lot like IBD. Even Crohn’s disease and ulcerative colitis can be hard to tell apart sometimes since they share so many of the same symptoms. The good news is that once doctors figure out exactly what’s going on, they can choose the right treatment to help you feel better and stay well.
At Digestive and Liver Center of Florida our GI specialists have a lot of experience diagnosing IBD in both kids and adults. They take time to listen, review your symptoms, and use the latest tests to get a clear picture of what’s happening inside your digestive system.



Tests That Help Diagnose IBD
When your doctor thinks IBD might be the cause of your symptoms, they may start with a few simple tests and then add others as needed.

Blood and stool tests
These are usually the first step. They can check for things like anemia (low red blood cells), infection, or signs that your body isn’t getting enough nutrients.

Colonoscopy

This is one of the most important tests for diagnosing IBD. Your doctor uses a thin, flexible tube with a tiny camera to look at the inside of your colon and the lower part of your small intestine. They can also take tiny tissue samples (called biopsies) to check for inflammation.

Flexible sigmoidoscopy
This test looks at the lower part of the colon—just the last section near the rectum. It helps your doctor see if there’s any irritation, ulcers, or bleeding, but it doesn’t show the whole colon.

Upper endoscopy
This test lets the doctor look at your esophagus, stomach, and the first part of your small intestine. These areas aren’t usually affected by Crohn’s, but sometimes they can be, and this test helps check for that.

Imaging tests

Sometimes your doctor will order scans to see parts of your small intestine that can’t be reached with a scope. These might include:

  • A small bowel follow-through (barium test)
  • CT enterography
  • MR enterography (MRE)

These tests show whether the small intestine is swollen, narrowed, or has other changes caused by Crohn’s disease.

Capsule endoscopy 

This one is pretty amazing—you actually swallow a tiny capsule that has a miniature camera inside! As it moves through your digestive tract, it takes thousands of pictures of your intestines. You wear a small recorder on your belt, and the camera naturally passes out later when you go to the bathroom.
