A little automation update for patients and referring physicians…
We have updated our website (DLCFL.com) so patients can electronically submit New Patient Forms (English & Spanish) and physicians or referral coordinators can submit Direct Access Forms. Forms are auto fillable PDFs that are sent via secure email.
